Rate One Fiber Broadband retains the authority to approve or reject this service termination request if it is not submitted by the registered account holder, or if the provided information and signature do not match those in the signed Application Form. This request must be made using the registered email address as stated in the application.
I also confirm that my Rate One Fiber Broadband account has no outstanding balance. If there is any outstanding amount, I understand and agree that this termination request will be considered invalid.
Any pending or uncleared final bill payment will result in the rejection of the device return, regardless of the delivery method or condition of the returned device.
If the registered phone number is unreachable for verification or clarification purposes, the termination request will be deemed incomplete and may be rejected.
